Judo throws fail for reasons that happen before the throw. If uke is not broken off balance, the strongest uchi-mata in the dojo turns into a lift and a stall. If the grip battle is lost, the entry never gets a chance to start. Seoi-nage entries go wrong in the turn and the depth of the drop, and a beat later the chance is gone. Then there is what happens after: a throw that lands short leaves you in a scramble, and whether that becomes a hold or a lost position depends on how fast you follow into newaza. None of this is easy to feel while you are doing it, and a coach running a full mat cannot watch every rep.

It checks your kuzushi, the timing and structure of the grip battle, the turn and depth of your uchi-mata and seoi-nage entries, and the transition from a landed or failed throw into groundwork.
